Where should my sitemap be?

Usually at /sitemap.xml, and listed in robots.txt with a "Sitemap:" line so every crawler can find it. Also submit it in Google Search Console.

Does every page need to be in the sitemap?

Every page you want in search should be, and only those. Leave out redirects, noindex pages and duplicates.

Does Google use changefreq and priority?

No. Google ignores both. It uses lastmod when the dates are accurate, so keep lastmod honest.
